Acting in a pro bono capacity on behalf of Advocate, Sam Louwers represented the bereaved family of Danny Lynch at a 2-day inquest before His Majesty’s Senior Coroner, Maria Voisin, regarding the death of Danny in the Bristol Royal Infirmary in November 2023.

The inquest heard from a number of clinicians who were involved in Danny’s care and described how he had come to require a tracheotomy. However, during this procedure complications arose which led to Danny going into cardiac arrest and developing a hypoxic brain injury.

As a result of these complications, Danny sadly passed away on 22 November surrounded by his family.

The coroner heard evidence throughout the inquest as to how the complications had arisen and subsequently led to the death.

Sam supported the family throughout and made closing submissions on their behalf as to the finding that the coroner may make.

The decision of the coroner was that this was to be considered as an accident and that the cause of Danny death began at the point of the cardiac arrest.

Sam has found it a real privilege to be able to support the family throughout this difficult process, and was assisted by Kirsty Partridge of Humphries Kerstetter.
